Form Mapping

When you generate Acord and other forms, information from specific policy fields in AMS360 appear in corresponding form fields.  AMS360 is designed so that each section of the Policy data entry form and Line of Business form roughly corresponds to section on the Acord forms. 

1. You can access these instructions in the AMS360 Online Manual as follows:

• When inside the Policy Information or within a Line of Business strike the F1 key to take you to the AMS360 Online manual (Click here for instructions)

1. From the Policy Information screen:

• Strike the F1 key and the AMS360 Help Online manual will open to the below section

• Scroll down towards the bottom of the page and you will find the link to Forms Mapping - Click here to see where data flows to the Acord and other forms

• Click on the link to take you to Forms Mapping instructions for the following sections:

  • Applicant Information Section (Acord 125)

  • Contact Names

  • Additional Premises Information (Acord 823)

  • Additional Named Insureds

1. If you click on the Forms with Images of the Policy Data Entry Section (Acord form (Output)) link you will see the Acord Form.  Click on the Area of the Acord Form you wish data to flow

• The Data Entry Section will appear to show you where to input the data in the data grid.  It also provides you with directions on where to access the fields in the data grid (Input).

2. If you click on the Policy Data Entry Sections with images of the form (Data Entry section (Input)) link you will see the Data Entry screen.  Click on the area of the data entry screen you wish to see where the data will flow (Output)

• The Acord Form will appear to show you to where the data you input in the data grid will flow to on the form (Output).

• Strike the F1 key and the AMS360 Help Online manual will open to the section for that line of business
